Why this check-in exists

Recovery is a journey with recognizable stages — from not yet seeing the problem, to getting ready, to taking action, to building skills, to sustaining a new life. No two journeys are alike, and people move back and forth between stages. This check-in helps you locate where you are today, honestly, and points you to the course in the REBOOT Recovery Course Series that best fits that place. It is a compass, not a diagnosis.

Safety first — read before you score

This is an educational self-assessment, not a medical or clinical instrument. If you are experiencing withdrawal symptoms (shaking, sweating, seizures, hallucinations), a medical or mental-health crisis, or thoughts of harming yourself, stop here and get professional help now — call your doctor, go to the nearest emergency room, or in the U.S. call or text 988. Detox from alcohol and some drugs can be dangerous without medical supervision. Courses come after safety.

How to score

For each statement, mark how true it is of you over the past 30 days: 0 = not true · 1 = a little true · 2 = mostly true · 3 = very true. Answer honestly — this only works if it describes the person you are today, not the person you want to be. Then press Score my check-in.

One more honest question — the lapse check

A relapse is a recognized stage of the journey, not the end of it. In FOCUS language: recalibrating is strength, not retreat.

Please answer every statement — still unanswered.

Where you likely are

Because you noted a recent lapse or relapse: whatever your scores say, restart at Course 1 · STABILIZE and rebuild the foundation — then move forward faster the second time. Recalibrating is strength, not retreat.

One size doesn't fit all. This result points you to a starting line, not a verdict. Different substances, histories, health realities, and support systems mean the right course — and the right pace — varies from person to person. Review your result with a counselor, sponsor, or clinician and resize it to fit: the framework holds, the numbers flex. Both formats are open to you — instructor-led in a group, or self-led at home with an accountability partner.

Make it a FOCUS goal: write down your stage and course, share them with one person, and set a 30-day review date. Scored · Shared · Scheduled — that's how a result becomes a beginning.

Print opens your printer dialog — choose “Save as PDF” there to keep a copy. Email opens your mail app with your results pre-filled; you choose who receives them. Your answers never leave this page otherwise.

Already in a course? Take the detailed course assessment

This check-in locates your starting point on the journey. Once you're inside a course, each course has its own detailed 30-day assessment — the same unit-by-unit standards its syllabus grades, scored against the 80% pass bar, with a Gate-readiness verdict and a remediation plan for your weakest unit.
